| 12 months | MMR, Varicella, Hep A, Flu if needed |
| 15 months | DTaP, HIB, PV13, Flu if needed |
| 18 months | Hep B, Flu if needed |
| 2 years | Hep A |
| 3 years | Developmental Exam |
| 4 years | DTaP, MMR, Varivax, IPV |
| 5 years | Developmental Exam |
| > 10 Years | Tdap, MCV4, HPV (Girls Only) |
Ages 5-10 years – Well checks are recommended every other year unless child is participating in sports. Sports physicals must be obtained on a yearly basis.
Ages 11 and above – Well checks are recommended every year.
